What to expect at your initial appointment
Please arrive for your appointment 15 minutes prior to the scheduled time.
Dr Kovac’s main rooms are located at Saint John of God (SJOG) Subiaco Hospital, Ground Floor, Suite 213, 25 McCourt Street, Subiaco WA 6008. Parking is available on site or Subiaco train station is located 750 meters from the hospital campus.
What should I bring to my initial appointment?
At the time of your first appointment, please bring the following with you:
A valid Referral letter from your GP, family physician or Specialist
Your Medicare card / Private Insurance card / Veteran Affairs Card
Your workers compensation or third-party claim details ie. Your employer details and Claim number
Any Military paperwork for defence personnel
Private Hospital Insurance details
Copies of recent results, x-rays, MRI’s, CT scans etc and any other relevant information such as operation reports from relevant previous surgeries. This may include CD’s, USB Flash drives and/or online logins.
A list of any current medications
Please wear comfortable clothing and shorts (or other appropriate clothing) for your appointment.
Fees
Dr Kovac works with all known health funds and therefore most appointments/procedures are billed directly to the insurance provider. There may be a gap for consultants and or procedures as it depends on the length and the complexity of the case. An estimate of any gap will be provided by our staff prior to a consultation or procedure.
For patients with overseas insurance or uninsured patients who wish to go privately the fees charged are at the level recommended by the Australian Medical Association.
The surgical fee will include post operative appointments within the immediate 6 weeks post operative period and patients are provided prescriptions and/or paperwork/certificates as required.
